...is simply a matter of operating the safety locks before pulling/turning the handle. They're usually of a plug type design that requires them to be pulled in before being pushed outwards, making it impossible if there's a pressure differential. .......AND any that I have seen that do open straight out - have a locking mechanism in place that prevents to door lock-pins being withdrawn when the aircraft is pressurised.
