Well, the reason could be hardware or software related. The abend shows
that the IDE driver IDEATA.HAM is involved. One of the biggest issues I
can see from the software point of view is that your server seems to be a
completely unpatched server. The latest SP for NetWare 4.11 was SP9. So
you are 9 support packs behind! Installing SP9 would therefore be a strong
recommendation for proceeding. However this would require the server to be
up and stable enough to survive through the SP install. Maybe it's just
the IDEATA.HAM driver that is the main source of problem and you could
manually update that file witht he SP9 version and see if that makes the
server stable enough to apply the full SP9.

http://download.novell.com/Download?buildid=AzcHqMnSuZQ~ is multilinguaƩ
and will install the correct files for your server language.

Honestly, it looks like your hardware has serious problems. Maybe you can
plug the hardisk into a different machine.
