Thank you for contacting Microsoft Online Community Support. It is Mark, a
SQL Server Engineer. I'm glad to assist you with the issue.
For this case, you indicated that when Ctrl-Shift-M, the Specify Values
for Template Parameters dialog box doesn't appear. Is it correct? If I
misunderstand anything, please tell me directly. It will help us to resolve
this issue quickly. Once the issue resolved, I'll appreciate your
verification.
In order to resolve the issue, I would like to explain the following
I) I have re-produce the issue and compare it with SQL Server 2005
Management Studio. It is confirmed that the "Ctrl-Shift-M " shortcut key
does not exist in SQL Server 2008 Management Studio. If you have concerns
on that, please send us a feedback via the
address( https://connect.microsoft.com/sql). Besides I'm consulting with our
dev team with the issue; if anything updates, we will public it.

That's why I assumed it was still there

Welcome, it is my pleasure to assist you with the techonical issue.
Through my test, it is confirmed that the Specify Values for Template
Parameters function still exist but the "Ctrl-Shift-M" shortcut key(call
the Specify Values for Template Parameters function) disappear in SSMS
2008. So to use Specify Values for Template Parameters function, we should
manually open it's dialog box. (Query ->Specify Values for Template
Parameters)
Based on the above, I'm consulting with our dev team. If anything updated,
I will email you. If it is convenient to you, could you please provide your
email address to me?
Besides, as you following description, does it mean that when you press the
"Ctrl-Shift-M", the Specify Values for Template Parameters dialog box
appear?

After consulting with our development team, the issue is confirmed as
by-design. It looks like Ctrl-Shift-M is no longer mapped to the menu item
for Specify Values for Template Parameters in SSMS 2008. If you have
concerns on that, please send a feedback via the
address( https://connect.microsoft.com/sql). Our develop team will prove it.

Take out the "online." part. Or, just continue the conversation here, so
everyone can benefit (and provide input). Did you try what Tibor suggested,
changing the keyboard scheme to 2000 and then back?
