First, the "Outlook Address Book" entry in the Address Book view will never
contain contacts, only references to contacts folders, just like you see.
The fact you see a "Contacts" entry that contains nothing indicates a
corrupt Outlook Address Book service. You should fix your Outlook Address
Book service to remove the first bogus Contacts entry. Exactly how depends
on Outlook version. For OL 2002/2003, it's Tools>E-mail Accounts>View or
change existing directories or address books>Next, select the OAB service,
click Change, and delete the first entry. Stop and restart Outlook. For
Outlook 2007, click Tools>Account Settings, and select the Address Book tab.
The rest of the process is similar.
