Assuming that you are not using Exchange as your mail server, your contacts
and all other Outlook data, plus the form definition, are stored in your .pst
file. See File | Data File Management for its location. If you copy that file
to the new machine, you'll have it all. Just make sure you use that .pst file
as the default store for the profile on the new machine.
You can also use the File | Save As command to save a backup copy of your
form as an .oft file. I recommend strongly that you get into the habit of
doing this whenever you make design changes.
I have crossposted this response to the
