I ended up figuring it out. The virtual settings were all ok, as were the
NTFS security settings. What it ended up being was the I_USR account. I
manually reset the password to a complex password, changed it to match in
all of the places, and then it was ok.
So, it was somehow corrupt or out of sync with the IIS metabase. When I was
looking in the log files it was showing 401 errors.

I had a major meltdown with my exchange server yesterday.
Had to perform a clean install of the OS windows 2003 STD, and then
re-install of Exchange setup /disasterrecovery
The machine is using the same name OWA, and I had a number of difficulties
getting the stores to mount again, but all that has been resolved, and the
messages are all back.
Now when I log into Outlook Web Access all of the images are missing, and
none of the scripts work.
I have followed Microsoft's recommendations for deleting and letting the
System Attendant re-create the virtual directories. Still no luck.
I have checked the NTFS permissions on the exchweb folder, and they are
the same as on another exchange box. I've checked and all of the images
exist where they should, and I can open them on the server. The path to the
missing images is correct as well.
This is what I see with Internet Explorer 6.0.
I don't know what else to check to try and resolve this.
