We found the latter worked very well for me. I got one of those memory
foam pillows (from Tesco Extra at Bar Hill) and it holds my head in a
much better position ... so long as I don't roll over onto my back ;-)
But the real solution is to lose weight, which I'm also working on ...
slowly.
