Description: As the Home Care Supervisor you will:

- Direct, co-ordinate, and evaluate the delivery of home health services
- Monitor nursing staff assignments, caseloads, service utilization, and team productivity
- Provide direct supervision to field staff, including on-site supervisory visits
- Ensure all documentation is timely and accurate, including OASIS, plans of care and daily visit documentation
- Participate and provide as requested training continuing education
- Participate in nursing orientation
- Evaluate and assure competency requirements are met for all clinical staff
- Identify appropriate staffing patterns and needs of assigned staff
- Provide branch manager with census, staffing, team productivity and other reports as requested
- Work on performance evaluations and disciplinary action of all assigned staff
- Coordinate appropriate communication and documentation between patients, agency, medical professionals, and referral sources
- Evaluate, monitor and suggest revisions to clinical documentation system
- Evaluate nursing documentation
- Assist with assigned performance improvement data collection
- Implement performance improvement plan as directed
- Ensure compliance with state and federal home health regulations
- Ensure compliance with extended home care policies and procedures
- Provide direct field supervision to team staff
- Demonstrate sound judgment by taking appropriate actions regarding suspected violation of corporate compliance regulations
- Report all suspected violations to supervisor, compliance officer or compliance hotline
- Perform other duties as assigned

Required Skills:
- 4+ years of relevant work experience
- New York State Registered Nurse license
