Description: Responsibilities for the Field Nurse Supervisor include:

- Instituting contact with physicians as necessary
- Conferring with patients physician in ascertaining the need for home care and in developing, implementing and revising the plan of care
- Performing initial patient assessment on those patients admitted to the agency for care
- Developing an individualized goal outcome plan of care
- Ensuring that physicians orders are obtained initially, reviewed as required and communicated appropriately
- Assessing the appropriateness/effectiveness of all paraprofessional caregivers on a regular basis and annually
- Submitting and maintaining accurate and timely documentation to client care
- Reviewing paraprofessional documentation
- Conducting training programs for PCAs and HHAs
- .Participating in orientation to policies and procedures for staff members
- Performing initial and annual competency testing on paraprofessional staff
- Acting as a mentor when appropriate for other health care workers
- Participating in quality assurance and performance improvement activities as assigned
- Monitoring patient/employee infections and maintain infection control log
- Monitoring reporting of complaints and occurrences related to clinical issues and maintaining complaint/occurrence book
- Performing other duties as assigned

Required Skills:
- 1+ years of experience as a Registered Nurse
- 1+ years of home care experience
- Associates degree in Nursing
- Valid New York State Registered Nurses license
