Description of Duties To oversee and manage day-to-day activities involving the Emblem Health Medical Home Project (high level initiative). This will include implementation of the project and direct oversight; this will require a high level of decision making. This involves coordinating the efforts of the complex functions and personnel across several Emblem departments, contractors and physician practices. * Maintenance and implementation of the Project Workplan as described in the ECI contract and in the Medical Home Project Plan. Manage timelines with accountable parties; facilitate progress towards completion. * Oversight of the physician practice recruitment effort. Physician outreach and contracting. * Oversight of the provision of performance data. The project director will work with analytic staff and contractor-Doc Insight to ensure that performance data is available in a timely way to participating practices. * Management of provider payments by Emblem Health for both controlled and supported practices. * Coordinate with applicable department(s) to ensure changes to the provider reimbursement are implemented and tracked per the provider agreement and achievement of the program goals. * Liaison to program evaluation contractor (the Tripp Center at the University of Connecticut). The project manager will work with the program evaluation contractor and the support contractor to coordinate project timing and to ensure that the evaluation contractor receives the necessary information. * Oversight of support contractor (ECI and Doc Insight). The project director will provide direction and oversight to the support to ensure that the ECI and Doc Insight vendors meet their contractual obligations * Develop interfaces between ECI and the applicable disease management and case management vendors as well as the internal HIP case management department. * Oversee the project with UM and OM staff. Develop workflows, program contacts and policies and procedures. * Manage the Project Operations Steering Committee and Project Advisory Committee including scheduling of meetings, ensuring minutes are taken at each meeting and coordinating the various agendas of the participants to stay focused on project objectives. * Lead and coordinate the weekly or as needed meetings to manage the day to day project details.
Requirements Knowledge Skills & Abilities: * This position requires a high level of decision making. Must have the skills to adjust priorities and be flexible in a dynamic work environment. * Candidate must have the ability to determine what issues can be resolved by position and what issues need to be raised to a Senior Management level; understand implications of all decisions and how other EmblemHealth areas are impacted. * Communicate decisions either through policies and procedures or other informal communications to any and all appropriate parties and offer solutions and explore options. Independent action and proactive problem solving is essential. Organizational/Project Management and Excellent oral and written communications skills. * Five years experience in Health Care Project Management. * Bachelors Degree in Health-Care or Business field. Masters Degree a plus but not required. * Proficiency in MS Word, Excel, MS PowerPoint, Visio; Knowledge of Q-Care and CPF systems.
