Position Description Job Summary: Director, Patient & Family Services. Flushing. Responsible for mgmt & delivery of ACS Patient & Family Services programs. Interacts directly with healthcare providers, patients & family caregivers; Social services referrals. Reqd: MA Counseling; CPE; 2 yrs. exp. job duties + coping w/cancer diagnosis, treatment & loss. The Director of Patient and Family Services represents the American Cancer Society as a leader of the Agency and reports directly to The Regional VP. This position is responsible for the management and delivery of the Priority Patient & Family Services educational programs: Look Good .Feel Better, I Can Cope and Patient Navigation. Identifies, cultivates and manages key relationships with the leading hospital cancer centers and community organizations in the Region. As a Community Mobilizer, you will be responsible for integrating and coordinating division-wide efforts to mobilize volunteers and communities, participate in the Eastern Divisions special events and vital outreach activities, serve as an expert resource for motivating staff and volunteers and establish a team atmosphere conducive to sharing our "best practices" and other resources as appropriate, and provide leadership and coach others in support of our mission, goals and strategic outcome. Responsible for meeting the goals and objectives established in your individual Performance Achievement Communication Tool (P.A.C.T.) Essential Duties and Responsibilities: Ensures consistency in the delivery of Patient & Family Services programs and services Collaborates with all Cancer Control Directors in delivery of divisional programs Co-manages the department Coordinators in regions with multiple Directors Conducts continual needs assessment throughout region to identify patient & family resources and gaps in the community Implement survivor-oriented educational programs. Recruit, train and maintain volunteers for regions survivor-oriented programs Interacts directly with patients and their families Intervenes and assumes responsibility for resolution when patient service calls reach a complicated or crisis level Build collaborative relationship with all ACS functions Build collaborative relationships with health care and community based agencies. Collaborate with Corporate Relations staff to cultivate relationships . Develop a corporate component for Community Action Plans Action Manage field supervision of an undergraduate or graduate student in social work. Some evening and weekend meetings and/or program participation required Participate in Making Strides Against Breast Cancer, Relay For Life and other ACS events/activities as appropriate Performs other duties as assigned Contacts and Relationships: Reports to the Regional VP 1. Position Requirements Qualifications and Key Competencies: BS in Social Work or Nursing, with a specialty in Oncology, Masters preferred. 3-5 years experience with program planning, development and evaluation of educational support programs for cancer patients and their families Proven management skills Ability to work with diverse populations Fine tuned communication skills, interpersonal, active listening and written Ability to prioritize and multi-task management of projects Strong networking skills Strong and effective verbal and writing communication and listening skills Computer proficiency Some Travel required Multi-lingual preferred Commitment to the mission and goals of the American Cancer Society
