SIX SESSIONS PER WEEK. Position has been re-posted. Brief Description of Duties: Provision of an Anticoagulation Management Service (AMS): The successful candidate will function as a Nurse Practitioner that provides care to patients from pediatrics to geriatrics that are on anticoagulation therapy. Anticoagulation management will be provided utilizing an interdisciplinary approach.

Responsibilities will include: Point Of Care Testing (POCT) of INR using Capillary whole blood via finger sticks. Compliance with Quality Control and Laboratory Procedures in accordance with regulatory requirements. Management of anticoagulation dosing and prescription attainment. Evaluation of patient specific risks and benefits to determine the appropriateness of therapy. Continuous systematic monitoring of patients INR results, diet, concomitant drug therapy and disease states. Ongoing education of the patient and other caregivers about anticoagulation therapy and the importance of self-care behavior leading to optimal outcomes. Communication with other healthcare practitioners involved in the care of patients.

The purpose of the Anticoagulation Management Service is to provide safe and effective management of anticoagulation therapy in a systematic, efficient and coordinated approach. The goals of the program are to decrease the incidence of complications and hospitalizations related to anticoagulation therapy; facilitate safe, prompt hospital discharge for patients on anticoagulation therapy; serve as a clinical resource for patients and healthcare providers; and provide quality control measures for improved care. DUTIES AND RESPONSIBILITIES ARE NOT LIMITED TO THE ABOVE POSITION DESCRIPTION Qualifications: NYS RN License required. MS in Nursing and NYS certification as a Nurse Practioner in Specialty area required. A minimum of two years post-licensure clinical Nursing experience preferred. BLS (Basic Life Support) certification requirementsmust be successfully completed within three months of employment. Verification of credentials and experience subject to Medical Board review and approval prior to appointment of the selected candidate.
