Description: Duties: -Responsible for performing admission history and physicals on all new clients. Documentation in the medical record consists of physical findings, treatment plan, written orders and prescriptions if indicated. -Responsible for ordering follow ups in the clinic with provider, physician or tertiary clinic. -Orders, interprets and follows up on lab tests and results. -Responsible for daily sick call list. - In onsite emergencies, the nurse practitioner will provide care and supervise the transfer of clients to emergency room as warranted. -Follows up on referrals to outside entities/specialty clinics. -Patient teaching and discharge planning. Hours are full time, Monday-Friday 9:00am-5:00pm. Education: Degree in nursing from accredited college/university; graduated from accredited or certified nurse practitioner program. Qualified by training, education and experience in the provision of medical care and assessment, in collaboration with a physician, of adults clients. Knowledge of substance abuse a must.
