Description: Title: Clinical Coordinator City: Bronx State: NY Min Salary: $80k Max Salary: $100k Job #: 24628 Description: GENERAL PURPOSE OF THE JOB: This position is responsible for performing and coordinating nursing care and services in a chronic-care facility for the end stage renal disease patient.
SUPERVISORY RESPONSIBILITIES: No No direct reports. Assists in the evaluation and counseling of facility nursing care staff under the guidance of the Facility Administrator. Participates in staff and patient scheduling and report on staff performance. Supervises day-to-day patient care of Patient Care Technicians and RNs.
ESSENTIAL DUTIES AND RESPONSIBILITIES: The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive. Patient Care Perform and analyze patient assessments: vital signs; laboratory data, note any signs of trauma as well as patients general physical and mental condition; review dialysis orders; document observations in a legible manner in patient chart or electronic medical records; report results to physician(s) as necessary Assess patient and family learning readiness; provide appropriate information on renal disease, prescribed dialysis treatment and medical regimen and their impact on the patients state of health and well-being as needed; participate in ongoing patient education; instruct patient on pre- and post-dialysis care Provide hemodialysis therapy: formulate a dialysis plan per physician orders and weight and rate to be ultra-filtrated; ensure safety measures; monitor patient vital signs during treatment; administer medications or other appropriate treatments; discontinue dialysis according to established procedure; document all activity appropriately Administer quality patient care in a considerate, respectful manner
Planning, Coordinating and Organizing
Supervise/monitor direct patient care administered by PCTs and RNs Review vital signs; initiation and termination of dialysis; documentation; physical assessment of all patients each treatment Ensure the transcription of all physicians orders are placed in patient chart Ensure documentation of all laboratory work orders Financial Management/Quality Assurance Ensure charges for dialysis are recorded according to company policy Complete flow sheets and charge vouchers Ensure availability of inventory and supplies; may oversee work of Inventory Technician and approve supply order if delegated to do so by FA Participate in quality assurance cost containment programs Comply with our policies and procedures in accordance with all state and federal regulations for patient care and services (CMS, HIPAA, OSHA, or other governing agencies) Comply with state and federal regulations of the Injury Prevention and Safety Training Program Work collaboratively with Clinical Services Specialist as needed
Other Duties Attend staff meetings and education activities Other duties as assigned Able to work overtime with little or no notice Read, understand and follow all The Company patient and employment policies and procedures Know facility emergency plans and demonstrate ability to act swiftly, confidently in an emergency
MINIMUM QUALIFICATIONS (Education, licenses, certifications, and experience required to fulfill the essential duties, include computer skills as required): Graduate of an accredited school of nursing Current RN license within state of practice Minimum one year of dialysis experience in outpatient unit and a minimum of 3 years patient care experience as a RN with demonstrated ability in clinical assessments Basic to intermediate computer skills; ability to use all required computer systems of The Company including but not limited to Outlook, SNAPPY, DUCK, etc. CNN certification preferred Current CPR certification required (or must be obtained within 30 days of employment) Clinical supervisory experience preferred; willingness, desire, and ability to provide leadership required Demonstrated ability to communicate effectively to a diverse audience with varying levels of understanding related to ESRD

However, in general, the position requires the following physical activities: Ability to lift minimum of 5 pounds to a maximum of 35 pounds unassisted and able to perform repetitive standing, sitting, stooping, walking, stretching, reaching, and use full range of body motions.
