Description: Under the direction of the Clinical Director, the pharmacist is responsible for providing drug information and assisting licensed providers to ensure optimal drug therapy for patients. Dispenses medications to ambulatory patients in accordance with applicable laws, regulations and standards of practice. Compounds, prepares, mixes, manufactures and dispenses medicines and preparations in accordance with pharmacy law and accepted standards of practice. Oversees the acquisition and storage of all drugs. Responsibilities: Review appropriateness of current medication therapy, including: Correct drug, dose, and frequency for indication/diagnosis, drug-drug interactions, therapeutic duplication, cost-effective route of administration, and correct therapy appropriate to the age of the patient. Provide drug information to physicians, nurses, and other health professionals, including drug inservices to various medical center disciplines as requested.. Perform and documents significant clinical findings and actions, such as interventions, appropriate calculations, adverse drug reactions, patient teaching, and pertinent pharmacy notes.. Interpretation of the orders, contacting the prescriber if clarification is necessary, and enter patient information on the correct profile, including correct drug, dose, frequency, route, dosage form, allergies, date of birth, gender, prescribing physician and other appropriate clinical date. Compounding, enter appropriate payment information for third party, cash or accounts receivable payment methods, dispense medication to patients and supervise technicians in the dispensing process, minimize the occurrence of medication errors via the evaluation of system sand processes. Check dispensed patient medication for accuracy according to departmental policy.. Complete duties of the in-charge pharmacist when in that role, supervision of pharmacy area for safety issues and effective workflow, prioritize workload tasks, review daily tasks and compounding needs, assign tasks, perform or delegate narcotic count.
