ok i'm not looking to crush my pills I'm trying to understand them.
i'm the worlds biggest fraidy kat about drugs. In fact I was on
Fentanyl 50 mcg which I asked to be taken off of cause it made me have
burning sensations and seemed to upset all my anxeity problems... so I
was switched to 20 mg twice a day of methadone. which I was on for
about 3 months. Yesterday morning I took my last dose of Methadone and
had huge anxiety attacks all day that I would have withdrawal... even
though I had been switched to the 20 mg extended release of
oxycontin.
Now here is the question...
If I take my pill at 8am I get 20 mg released then... well then do I
have another 20mg released say in 2 hours or 3?

Take it as it was Rx'ed and do not screw around with it.
it is a 20 it has 20. if you screw around it will not work as promised and
then you will be really in a mess of pain

First off, the 20 mg ER means that you get that 20 mg spread out over
several hours. There are teeny little holes in the casing of the
tablet that allow a small amount of the med to leach out at a time and
that is what makes it extended release. You are not getting a full 20
mg every few hours. Each pill, as another noter said, only contains
the amount stated on the bottle.
Secondly, I'm really not sure but I think the amount of oxycontin they
are giving you to switch to is too low according to the conversion
charts. You may end up in some bad pain unless you have been
prescribed breakthrough meds. How often can you take this
medication? Are they trying to wean you off pain medication
completely? Is this being handled by a pain management physician or a
GP? Sometimes they don't understand the conversion rates and you will
end up paying the price.
Can you give some more information on how often you took the Methadone
and how often you're taking the Oxycontin?

Blinded and Joe... I would enever crush my pills... the reason I
brought that up is becuase it says if you crush your pills you will
get too much of the med at once... and i know for a fact 20 mg of
oxycodone is not that big of a deal... which is what leads me to
beleieve you get more than 20 mg in the pill... the pill is designed
to keep you at a 20 mg level for 12 hours... I understand better
because I asked my pharmacist. She said you definitely dont get like 5
mg then 5 mg then 5mg... you start off wiuth 20 mg right away and then
when it comes time to release again you get another 20. Otherwise this
pill wouldn't work.
and yes I go to pain management... but when i swtched my pain went
from about 7 to 9.

get 20 mg right away and then the pill is designed to keep your body
level at 20 mg for the next 12 hours... and she said she didn't know
how often it released or how much was in there... but there really was
more than 20 mg in the pill.

and I haven't had any withdrawal except for sweating.

No that's not correct. A 20mg tablet of Oxycontin contains a total of
20mg oxycodone in and extended release matrix.

a common misunderstanding i'mafraid - even for some professionals.
from the purdue pharma clinical fact sheet:
"The tablet strengths describe the amount of oxycodone per tablet as the
hydrochloride salt."
you have to first know that dosages start down at the 5mg range for
immediate release
the slow release forms are supplied in 10 mg, 20 mg, 40 mg, 80 mg, and
160 mg tablet strengths for oral administration.
- so a 20 er/hci is a slow release of the 20 over the total time span.
