I downloaded the new patch, but have only been playing SP after a long hiatus. On my first trip to the Hell Hellforge, I was faced with five (5!) boss packs at the forge, three rockworms, an Urdar, and an Abyss Knight groups. The boss Urdar and the Rockworms were basically unkillable with 10 lvl 27 skellies, 10 revives, and a merc dealing medium low damage. I ended up by leading the bosses

Somewhere on teh intarweb "duird_ESCNL" typed: Playing in Hell is very intensive.. and The Counties drop me a El and Eld rune. Next from box goes weapon of IK. Nice. I have multiple CD keys and PCs. Last night I was doing a few Nilly runs with my strafer and I found a nice three socket hat. I decided that I'd try a "Delerium" runeword as I had the runes and had never made one

That's more a balancing issue than a bug - IIRC what happened there was that the multiplier (basically %% of max monster life) used to determine how much damage an FE explosion causes is not as
