This can happen for two reasons 1. A connectivity problem happened between DPM and protected server 2. Agent on the protected server crashed. (To verify this check if there are any cab files under the folder %%windir%%\PCHealth\ErrorRep\QSignOff) If it is due to 1, you can re-run the consistency check to fix the issue. If the failure happened due to issue 2, please open a case with Microsoft
