When I had my first panic attack at age 12 I thought I was going insane (felt like I was leaving my body.) The subsequent panic attacks were all about fear of going insane. It caused heavy disassociation after the PA and I wouldn't even want to look at myself in the mirror (like I was seeing a "crazy person.")
